    * The existence of a comprehensive medical history, including the results of each visit
    * A description of all surgical operations
    * Treatment of papers and daily progress reports for all inmate

    * All results of tests, including blood tests and X-ray and ultrasound examinations

How do I know if something is missing from my dog on the documents? If possible, try to visit your old practice in person and asked to see your dog the original file.
